What is Semax?
Semax is built from the ACTH(4-7) fragment of adrenocorticotropic hormone, extended with a Pro-Gly-Pro sequence that improves metabolic stability. Despite its ACTH lineage, it does not retain the hormone’s corticotropic (adrenal-stimulating) activity.
How it is understood to work
Semax is studied for its ability to increase expression of brain-derived neurotrophic factor (BDNF) and its TrkB receptor, along with modulation of dopaminergic and serotonergic signaling and antioxidant activity in models of reduced blood flow to brain tissue.
Research status
It is approved and marketed as a nasal-spray medicine in Russia for conditions including stroke recovery and certain cognitive complaints, though it has not received FDA or EMA approval and is not an approved medicine in the US or EU. Outside that regulatory context, it is studied preclinically for cognition, memory, attention and neuroprotection.

Handling & storage
Supplied as a lyophilized powder; store unopened vials at -20 C protected from light, and after reconstitution with bacteriostatic water keep refrigerated and use within the timeframe set by the study protocol.

Questions
Is Semax the same as ACTH, and does it affect cortisol?
No. Although derived from an ACTH fragment, Semax’s structural modifications remove the corticotropic activity responsible for stimulating adrenal cortisol production, which is why it is studied for CNS effects rather than endocrine ones.
Is Semax an approved medicine anywhere?
Yes, it is approved and sold as a prescription nasal spray in Russia, but it has not been approved by the FDA or EMA, so its regulatory status differs substantially between regions.
